SALINE or SILICONE:
This is typically the first question asked at the consultation. Patients come in with questions regarding the difference between the two types. We have both types available during the consultation for patients to feel the difference. Saline, or salt-water, was the primary material used for primary breast augmentation surgery from 1993 to 2005 due to the moratorium on silicone implants. There are pros and cons to using either saline or silicone. Silicone and saline are equally safe. Both have a soft silicone shell. Silicone has a more realistic feel while saline can give more of a projected look. If there is an implant rupture, saline will deflate immediately and will be very easy to identify, while silicone is more subtle. An MRI is usually required to detect a leak with a silicone filled implant. When female plastic surgeons were surveyed as to the implants they most prefer, silicone was preferred. ABOVE OR BELOW THE PECTORALIS MAJOR MUSCLE:
The decision to go above or below the pectoralis major muscle is another option that will be discussed at your consultation. A majority of patients have the best outcome with the implant placed below the muscle. Historically, this will reduce the incidence of capsular contracture compared to placing a smooth implant above the muscle. If you have enough tissue in the upper part of the breast to cover an implant, you may be a candidate for the implant placed above the muscle. When the implant is placed above the muscle, Dr. Richards uses a textured implant. Recent research has shown that the capsular contracture rate of placing a textured implant above the muscle is similar to placing an implant below the muscle. The recovery is much faster when placing the implant above the muscle, but not every patient is a good candidate for going above the muscle. Dr. Richards will help guide you through this very important decision.*
BREAST IMPLANT SIZES:
Breast implants come in a range of varying sizes. The average implant size in the Las Vegas area is approximately 400cc while other parts of the country average in the 300-350cc range. Silicone implants can go as large as 800cc while Saline implants as large as 1200cc. An implant size is unique to each individual. Your breasts will be measured by Dr. Richards to help determine the perfect size. Your height, weight, current cup size, breast width, and breast measurements all help to aid in choosing the correct size. Dr. Richards uses 3D imaging combined with a sizing system to help you choose the perfect sized implant.*
SHAPES AND TEXTURING:
Smooth and round implants are most commonly used in a majority of patients. The implants move around the pocket like a natural breast. Textured implants are used if the implant is placed above the muscle. Recent research has shown that placing a textured implant above the muscle has reduced incidence of capsular contracture. Shaped implants can be used in select patients. Dr. Richards can show you these implants to help you select the proper implant.*
INCISION PLACEMENT:
After assessing your breast size, shape, and composition, Dr. Richards will determine whether the implant should be placed between the chest muscle and breast tissue (submammary/subglandular placement) or under the chest muscle (submuscular). He will also select the optimal incision technique, from one of two most popular options:
• Inframammary incision: below the curve of the breast
• Periareolar incision: around the lower half of the areola
BREAST AUGMENTATION PROCEDURE AND RECOVERY

Breast augmentation surgery is performed in an outpatient setting under general anesthesia, and typically lasts between 1-2 hours, depending on the specific surgical plan and whether ancillary procedures (i.e. breast lift) are performed at the same time. You will need to arrange for someone to drive you home after surgery, and to be with you for a day or two after your procedure, as you will be on pain medication and unable to drive. Your follow-up visits with Dr. Richards will be scheduled for one day after surgery, one week, three weeks, six weeks, and three months after your breast augmentation.

BREAST AUGMENTATION RECOVERY
Although the exact length of the post-breast augmentation recovery period varies from individual to individual, the average duration is 2-4 weeks. You will be given prescription pain medication to manage any discomfort, and will be able to gradually resume your normal activities after 1-2 weeks. Strenuous exercise or movements involving the arm and chest muscles should be avoided for four weeks. The visible results of your breast augmentation procedure will be noticeable immediately after surgery, and will continue to improve gradually over the next few weeks as any lingering swelling and bruising subsides.*
